Findings: The facility failed to appropriately care plan for the Alleged Victim (AV). AV moved into the facility early March, however, there was no care plan in place until mid June. AV was a fall known fall risk, as he/she had 23 documented falls with and without injury, placing him/her at a risk for serious harm. The facility's failures are a violation of resident rights, are considered neglect of care and constitute abuse.
Sanction: ALFCP20-00742 $500.00 fine assessed
