Findings: The facility failed to provide a safe environment for the Alleged Victim (AV). There was an altercation between AV and Witness #1 (W1) where W1 scratched AV when he/she grabbed at AV. Facility staff were to supervise W1 while he/she was in the common area to ensure resident safety. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
