Findings: The Alleged Victim (AV) was care planned to be full assistance with smoking and was to be wearing a smoking apron while smoking, however, on or about April 9, 2020, Alleged Perpetrator 2 (AP2) failed to place the smoking apron on AV while he/she was smoking, resulting in embers from AV's cigarette falling into his/her shirt, causing a burn to AV's chest. AP2's actions are considered neglect of care and constitute abuse. The facility failed to ensure care plans were followed which is a violation of Oregon Administrative Rules.
