Findings: Alleged Perpetrator 2 (AP2) failed to safely assist Alleged Victim (AV) in a transfer which resulted in AV falling. AV was transferred to the hospital for treatment and was diagnosed with a left leg fracture. AP2 failed to follow AV's care plan which is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment for AV which resulted in AV getting an injury. The facility is responsible for the supervision, training, and overall conduct of staff when staff are acting within the scope of his/her employment duties. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
