Findings: Witness 1 (W1) and Alleged Victim (AV) were care planned for staff to keep them separated at all times due to previous resident to resident altercations. On or about June 20, 2020, AV was found in W1's room. AV was on the floor with W1 standing over his/her with a broken commode leg, hitting AV. AV sustained several abrasions and had a bite mike of hie/her shoulder. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
