Findings: The facility failed to assess, intervene, and monitor when the Alleged Victim (AV) experienced a uncontrollable back pain and was sent to the ER. Upon AV's return to the facility, there is insufficient documentation to show that the facility assessed AV upon return or checked on by staff. A few hours after AV's return, AV was found on the floor, had hit his/her head and was sent to the ER. The facilities failures placed AV at a risk of serious harm and caused unreasonable discomfort. These failures are a violation of resident rights, are considered neglect of care and constitute abuse.
