Findings: Alleged Victim (AV) requires staff assistance to use the restroom. On or about June 14, 2020, AV was offered assistance to the restroom at approximately 7:00am which AV declined. AV used his/her call light to request assistance to the restroom at 7:13am. Staff did not respond to AV's call light for approximately 76.33 minutes. During that time AV had an accident in his/her clothing while waiting on staff for assistance causing AV unreasonable discomfort. The facility's failure is a violation of resident rights, is neglect of care and constitutes abuse.
