Findings: The facility failed to appropriately care plan for Alleged Victim's (AV) known risk of attempting to self transfer. AV is care planned for a two person hoyer lift for all transfers; however, AV often attempts to self transfer without assistance and this was known by facility staff. AV suffered from three (3) falls between February 29, 2020 and June 22, 2020 due to AV attempting to self-transfer without assistance. An investigation determined that AV's care plan did not have adequate interventions in place to help minimize AV's risk of self transferring. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
