Findings: The facility failed to provide a safe environment for Alleged Victim (AV) regarding his/her risk of falls. On or about June 22, 2020, AV fell and was sent to the hospital for treatment and was diagnosed with a hip fracture. The facility failed to provide basic care and/or services necessary to maintain AV’s health and safety resulting in a fall with fractured hip. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
