Findings: Alleged Perpetrator 2 failed to provide necessary care and services to maintain the health and safety of the Alleged Victim (AV). AP2 did not notice that the AV's left arm was sticking out past the edge of the wheelchair as they went through the doorway resulting in minor injury. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect the AV from neglect, which is a violation of Oregon Administrative Rules.
