Findings: The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. AV had a fall in his/her room trying to change his/her incontinence pad, was transported to the hospital for treatment and was diagnosed with a lumbar fracture. The facility's failure to provide a higher level of care is a violation of resident rights, is considered neglect of care and constitutes abuse.
