Findings: Alleged Victim (AV) is a known fall risk and is care planned to have a non-slip pad in his/her wheelchair. On or about June 25, 2020, AV was found on the floor in the common area. AV stated he/she slipped out of his/her wheelchair. At the time of the incident the non-slip pad was not in place. The facility failed to follow AV's care plan putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP20-01083 $500.00 fine assessed
