Findings: Alleged Victim (AV) relies on the facility for care. On or about June 17, 2020, AV had a significant change of condition. AV had a urinalysis completed both in the hospital and in the facility on June 17, 2020, both results were negative. On or about June 20, 2020, AV complained about burning and urgency while urinating. On or about June 21, 2020, AV reported increased frequency in urination. On or about June 24, 2020, AV had brownish-pink foul smelling discharge coming from his/her urethra. AV was transported to the hospital and diagnosed with a infection of the bladder and sepsis. The facility failed to respond to AV's significant change of condition on June 20, 2020, which is a violation of resident rights, is neglect of care and constitutes abuse.
