Findings: The facility failed to properly plan care for Alleged Victim's (AV) risk of falls. Between May 19, 2020 and July 4, 2020, AV had approximately 18 unwitnessed falls. On July 1, 2020 the facility determined AV to be unsafe to reside in facility due to AV's need for higher level of care due to his/her fall risk. An investigation determined that AV had approximately 3 falls after time of being determined to need a higher level of care. The facility's failure is a violation of resident rights, is considered neglect of care which constitutes abuse.
