Findings: Alleged Victim (AV) has cognitive impairment and dementia. AV is not always orientated to person, place, or time. The facility recognized that AV needed increased level of care. On or about July 5, 2020, AV exited the facility without staff assistance, and was picked up by a stranger approximately two miles away from the facility. The facility failed to care plan according to AV's increased level of care needs putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP20-01031 $375.00 fine assessed
