- The allegation that the facility failed to provide a safe environment for Alleged Victim (AV) by failing to protect AV from repeated falls due to facility staff letting AV ambulate with AV's walker independently when AV is care planned for stand-by assist with gait belt support from staff. The facility's failure is a violation of resident rights is considered neglect of care and constitutes abuse.
