Findings: On or about July 11, 2020, AV pulled his/her call light for assistance with a bed change/peri care. A staff, who was in training, was instructed by his/her trainer that AV was a one person assist and was given the ok to provide cares alone. While staff was providing care AV rolled off the side of the bed sustaining injuries to his/her hands, knees, elbows, big toe, and reported pain. AV is care planned a two person assist for incontinence care at the time of the incident. Staff were not aware AV was a two person assist due to verbally being trained by former facility management incorrectly. The facility failed to follow AV's care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP20-01139 $250.00 fine assessed
