Findings: The facility failed to provide the basic care or services necessary to maintain the health and safety of Alleged Victim (AV) when that failure lead to skin tears. AV was found to have skin tears as a result of having long nails. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse. Alleged Perpetrator 2 (AP2) neglected AV by failing to follow AV's service plan resulting in harm to AV. AV's care plan provides instruction that only a licensed nurse may cut AV's nails and facility staff are only to file AV's nails. AP2 failed to read and follow AV's service plan prior to trimming AV's nails. AP2's actions is considered neglect of care and constitutes abuse.
