Findings: The facility failed to properly plan care for Alleged Victim (AV) which resulted in AV's basic care needs not being met. On or about July 10, 2020, AV reported to facility staff that he/she found blood from his/her wounds on his/her feet. The facility notified AV's primary care physician and a medical appointment was scheduled for July 20, 2020. Because of AV's wounds, AV was unable to transfer independently and spent most of his/her time reclining in his/her chair and bed. At AV's medical appointment, AV was discovered to be weak and dehydrated and was sent to the hospital for treatment. AV was diagnosed as being septic by hospital staff and was observed with pressure wounds covering nearly the entirety of AV's back and buttocks and had pus leaking from his/her catheter insertion site. AV's health declined in the ten days prior to his/her hospitalization due to facility failing to properly plan care for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
