Findings: On or about June 26, 2020 and July 13, 2020 a medication reconciliation list was sent to the facility. The medication reconciliation revealed concerns/questions around AV's blood thinner medication, blood pressure medications and insulin. An addition five issues were noted involving eye drops, docusate, hydrophilic/Euceren cream, lidocain patch, and enema. AV's blood thinning medication is listed twice in the medication administration record, once under the generic name and once under the brand name, both are signed as given on eight different occasions. The pharmacist talked with the facility on or about July 13, 2020 about the medication concerns. The pharmacist also faxed information to the facility to reiterate in writing the concerns. The facility was faxing documents to the wrong fax number. The facility failed to provide a safe medication administration system placing AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
