Findings: On or about June 30, 2020, the facility failed to provide a safe environment for Alleged Victim (AV) resulting in AV and Witness 1 (W1) having a resident-to-resident altercation in which W1 hit AV in the face. The facility staff failed to follow W1's care plan when W1 showed signs of aggressive behaviors on the day of the incident which placed AV at risk for potential harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
