Findings: Alleged Victim (AV) had a known history of falls. AV experienced falls on or about May 5th, May 10th, May 18th, June 20th, July 5th and July 7, 2020. The facility put some interventions in place however failed to put any new interventions in place after the falls on June 20th, July 5th and July 7, 2020 causing AV repeated unreasonable discomfort. The facility failed to care plan and implement interventions according to AV's history which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP20-00661 $500.00 fine assessed
