Findings: The facility failed to develop or maintain a comprehensive care plan for Alleged Victim’s (AV) risk for falls that included fall intervention planning. An investigation determined that AV suffered from two falls on December 22, 2019 and April 4, 2020 in which AV’s care plan documentation did not have updates regarding fall prevention/intervention. AV fell on June 21, 2020 and required hospitalization. The facility's failure to properly care plan for AV is a violation of resident rights, is considered neglect of care, and constitutes abuse.
Sanction: ALFCP21-01293 $500.00 fine assessed
