Findings: On or about July 20, 2020, Alleged Victim (AV) suffered an unwitnessed fall in his/her room. AV had complained of dizziness and nausea prior to this fall and complained of headaches, dizziness, and nausea in the two days after the fall. AV was transported to the hospital two (2) days after the fall and was diagnosed with a concussion and a spinal compression fracture. An investigation determined the facility failed to follow AV's care plan to provide assistance with ambulation and transfers and failed to provide the interventions and basic care AV required to ambulate safely. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-01199 $1500.00 fine assessed
