Findings: On or about July 27, 2020, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation in which W1 hit AV when AV wandered into W1's room. An investigation determined that W1 and AV had a prior resident-to-resident altercation on or about July 11, 2020 in which AV wandered into W1's room and W1 hit AV on his/her back, causing redness and pain to AV. The facility failed to provide a safe environment for AV by failing to provide supervision for his/her safety which is a violation of resident rights, is considered neglect of care and constitutes abuse.
