Findings: On or about July 30, 2020, Alleged Victim (AV) and Witness 1 (W1) had an altercation that caused injury to both AV and W1. An investigation determined that AV and W1 had prior altercations with each other and that staff failed to keep an eye on AV at time of incident. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
