Findings: On or about July 30, 2020, Alleged Victim (AV) and Witness 1 (W1) had an altercation that caused injury to both AV and W1. AV and W1 have a history of engaging in altercations with each other. On the day of the incident, staff failed to keep an eye on W1 which lead to the incident. The facility failed to provide a safe environment for Alleged Victim (AV) which is a violation of resident rights, is considered neglect of care and constitutes abuse.
