Findings: On or about July 31, 2020, Witness 1 (W1) ran out of his/her room and into the hallway after resisting assistance with his/her Activities of Daily Living and showing signs of agitation. W1 grabbed Alleged Victim (AV) by AV's shirt and shook AV. As a result, AV suffered from scratches to his/her chest after the incident. AV is care planned to be supervised to prevent altercations. The facility's failure to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-01674 $375.00 fine assessed
