Findings: The facility failed to provide a safe environment for the Alleged Victim (AV) during a transfer with a sit to stand device. On or about August 2, 2020, the AV suffered a skin tear to his/her left leg. It was determined that the skin tear was about the same height as a plastic portion of the sit to stand. The facility failed to provide a safe environment to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-01232 $188.00 fine assessed
