Findings: The facility failed to appropriately care plan and provide care for the Alleged Victim (AV) after his/her change of condition. AV was having issues eating and swallowing an was sent to the hospital for aspiration. The facility failed to update his/her care plan, causing him/her to have to visit the hospital multiple times, as staff were not aware of the issue and were not caring for AV as he/she should have been cared for. Staff were not assisting AV as they should have, AV's condition worsened, causing unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
