Findings: The alleged Perpetrator 2 failed to provide appropriate services to the Alleged Victim (AV) and follow his/her care plan regarding toileting. The failure resulted in the AV being left in a wet brief for an unreasonable amount of time, causing loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect AV from neglect, which is a violation of Oregon Administrative Rules.
