Findings: The facility failed to provide multiple services for the Alleged Victim (AV) to ensure his/her comfort and safety. Witness #2, on two different occasions, visited AV and found him/her without his her oxygen. One of the visits, AV's oxygen level was down to 69%. The AV also hadn't received help with oral care or received a shower in two weeks. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
