Findings: Alleged Victim (AV) had a known history of exit seeking behavior. On or about July 9, 2020, AV was found by the police department in the facility. It is suspected that AV left the building through an outside gate that was left unsecured. The facility failed to provide a safe environment putting AV at risk for harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
