Findings: The facility failed to follow Witness 1's (W1) care plan to provide supervision and redirection according to his/her known behaviors. The failure resulted in W1 walking over to the Alleged Victim and hitting him/her over the head with an object causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
