Findings: The facility failed to follow the Alleged Victim's (AV) care plan to assist him/her into night clothes and into bed each night. On multiple occasions, AV was found in his/her chair or sofa in his/her day clothes. On or about August 6, 2020 on night shift, AV was found sitting in his/her day clothes in his/her chair and was unresponsive. AV was transported to the hospital and diagnosed with a urinary tract infection. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
