- Findings
- The facility failed to follow Alleged Victim's (AV) care plan which resulted in AV sustaining an undesirable weight loss. An investigation determined AV's weight was not documented between March 2020 and August 2020. AV is care planned to receive weekly weight monitoring effective February 2020. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
