Findings: Alleged Victim (AV) is care planned for assistance with incontinence care, hygiene, and has bed sores. Staff are applying cream to AV's bedsores. Through record review it was determined that AV was not being cleaned properly before applying new cream. AV was care planned for staff to notify the LN when nail care was needed so an outside provider appointment could be made. AV's nails were reported to be long and dirty with no documented response on how the facility was coordinating care for AV's nails. AV is care planned for incontinence care and staff to empty his/her trash can at every brief change. AV 's briefs were noted to be soiled a lot of the time and his/her room had a strong urine odor. The facility failed to follow AV's care plan and provide necessary services, putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
