Findings: Alleged Victim (AV) had a history of suicidal ideation/attempts and suicidal statements as well a self-harming behaviors and has interventions for staff to follow in his/her care plan when displaying these behaviors. On or about August 20, 2020, AV was making suicidal statements and statements regarding wanting to be with his/her adult child. Facility staff did not know how to respond to AV making these statements and did not report AV's actions to management putting AV at risk for harm. The facility failed to ensure AV's care plan was followed, which is a violation of resident rights, is neglect of care and constitutes abuse.
