Findings: The facility failed to provide appropriate services and respond to the Alleged Victim’s (AV) change of condition. The failure resulted in AV being found in poor condition, was sent to the hospital, where it was discovered she/he had open wounds and poor hygiene, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
