Findings: The facility failed to ensure the Alleged Victim's (AV) care needs were being met due to a lack of service planning for catheter care. AV pulled his/her catheter out many times without the facility making any changes to his/her care plan or increased monitoring of AV to ensure AV was not pulling his/her catheter out. AV's pulling out the catheter caused undo pain and suffering to the AV. The AV was transferred to a higher level of care with available 1:1 care as needed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
