Findings: Alleged Victim (AV) has a significant history of ingesting or attempting to ingest inedible and potentially harmful objects and/or substances. On or about September 2, 2020, AV ingested an unknown quantity of an unknown resident's barrier cream which exposed AV to potential harm. The facility failed to ensure that potentially harmful substances were secure and also failed to provide AV with appropriate supervision, based on AV's known history of ingesting potentially harmful substances. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
