Findings: Alleged Victim (AV) was a known fall risk. Between July 1, 2020 through September 23, 2020, AV experienced four falls, resulting in injuries, when AV was attempting to get out of bed to use the restroom. The facility failed to care plan to mitigate AV's risk of injuries due to falling while attempting to get out of bed to use the restroom, which is a violation of resident rights, is neglect of care and constitutes abuse.
