Findings: The first week of August 2020, AV was diagnosed with a virus. Prior to AV's diagnoses AV was independent with activities of daily living. From August 6, 2020 through August 18, 2020, AV was documented as having increased pain, lethargy, an inability to transfer independently, an inability to toilet independently, and decreased fluid and food intake. On or about September 6, 2020, AV was found on the floor of his/her room with injuries and was transported to the hospital for treatment. The facility failed to put in reasonable interventions in place to address AV's serious symptoms related to a viral infection, which is a violation of resident rights, is neglect of care and constitutes abuse.
