Findings: Alleged Victim (AV) is care planned for staff to use a gait belt when assisting AV with transfers. On or about September 11, 2020, Alleged Perpetrator 2 (AP2) transferred AV without the use of a gait belt and AV fell sustaining a skin tear. AP2 was given the gait belt but was not given physical training, only verbal training, on how to properly use the gait belt. Additionally AP2 was not fully trained on AV's care plan therefore AP2 did not know to use a gait belt when transferring AV. The facility failed to ensure a qualified caregiver was present and that AV's care plan was followed which is a violation of resident rights, is neglect of care and constitutes abuse.
