Findings: Alleged Victim has a known history of falls. Between July 2nd, through August 30th, 2020, AV had sustained falls or had been found on the ground approximately six (6) occasions. On or about September 11, 2020, AV had an unwitnessed fall that resulted in a hematoma, a fracture to his/her left hand, and a compression fracture to his/her T12 vertebra. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
