- Findings
- The facility failed to properly follow physician's orders and inappropriately used bed rails on the Alleged Victim's (AV) bed. The failure resulted in Av's leg getting stuck in the bed rail, for the second time causing unreasonable discomfort and bruising, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
