Findings: On or about September 15, 2020, Alleged Victim (AV) fell when staff was assisting him/her with a transfer from his/her wheelchair to his/her bed. An investigation determined that facility staff transferred AV from his/her wheelchair to his/her bed without using AV's gait belt. AV is care planned for full assist by facility staff with all transfers using a gait belt. The facility's failure to follow AV's care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-01313 $375.00 fine assessed
