Findings: The Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim's (AV) care plan to ensure his/her chair is tilted back and the leg rests are in place to help avoid falls and injury. The failure resulted in AV experiencing an unwitnessed fall causing risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect AV from neglect, which is a violation of Oregon Administrative Rules.
