- The facility failed to provide a safe medication administration system to ensure Alleged Victim’s (AV) medications were administered as ordered. The facility's failure resulted in AV being admitted to the hospital due to AV not receiving his/her medication as ordered, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
