Findings: The facility failed to provide services and a safe medication administration system to ensure the Alleged Victim's (AV) medication was available to administer when his/her incontinence brief was changed and failed to ensure AV received showers on a timely basis. He/she went without a shower for approximately 13 days and never received his/her medication during his/her time at the facility, placing AV at a risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
